Insurance
We accept most major insurance plans and can assist you in understanding what you physical therapy benefit is.
- Blue Cross Blue Shield
- Vermont Managed Care (VMC)
- MVP
- Cigna/Orthonet
- Medicare, and supplementary insurance plans like AARP
- Green Mountain Care/VT Medicaid
- Auto and worker’s comp
If you do not have insurance, or have a high deductible, you may come as a cash paying client. Your initial evaluation will cost $135 and each appointment thereafter will cost $80.
Hours
Appointments are available Monday-Friday, 7:30am – 6:00pm. If these times do not work for you, please let us know. We are happy to try and accommodate all our patients schedules. Each appointment is 55 minutes long, so you get maximum treatment time.
New Patient Forms
Save time at your first appointment and fill out our new patient forms. You will provide us with your contact information, insurance information, brief health history, and signatures. At your first visit, be prepared with your insurance card, doctor referral (if needed) and co-pay.
If you need your records sent to us before your first visit, please fax them to (802) 863-9922.
Directions
We are conveniently located in the Innovation Center (the old General Dynamics building), off of Pine Street on Lakeside Avenue. When you reach our building at 128 Lakeside, turn right at the first driveway and park anywhere in the large parking lot. The main entrance is on this side of the building (the North side).
